Gateway for sensory, motor, and limbic integration — thalamus, hypothalamus, epithalamus, subthalamus.

🔹 Thalamus

relay hub The thalamus is the main gateway for ascending sensory information (except olfaction) en route to the cortex. It also integrates motor signals from basal ganglia and cerebellum and modulates arousal.

🧬 Thalamic pain syndrome – lesion of VPL/VPM → contralateral burning pain, loss of vibratory sense, gait ataxia. Resistant to analgesics.
Wernicke-Korsakoff: thiamine deficiency → degeneration of dorsomedial nucleus, mammillary bodies, hippocampus, cerebellar vermis. Anterograde + retrograde amnesia, confabulation.

🔸 Hypothalamus

homeostasis Master regulator of autonomic, endocrine, and limbic functions. Nuclei are organized into zones: anterior, tuberal, posterior, lateral, and preoptic.

Zones & nuclei

  • Lateral: feeding center; lesion → starvation
  • Ventromedial: satiety; lesion → hyperphagia, obesity
  • Suprachiasmatic: circadian rhythm (retinal input)
  • Supraoptic / PVN: ADH + oxytocin; lesion → diabetes insipidus
  • Arcuate: releasing/inhibiting factors; dopamine (PIF)
  • Anterior: heat dissipation; lesion → hyperthermia
  • Posterior: heat conservation; lesion → poikilothermia
  • Preoptic: gonadal regulation; lesion → sexual arrest / amenorrhea
  • Dorsomedial: stimulation → savage behavior

🧬 Diabetes insipidus: lesion of supraoptic/PVN → polydipsia, polyuria (lack of ADH).
Korsakoff syndrome: mammillary body damage → amnesia, confabulation.
Arcuate nucleus: dopaminergic neurons inhibit prolactin; lesion → galactorrhea & amenorrhea.
Temperature: anterior hypothalamus → heat dissipation (parasympathetic); posterior → heat conservation (sympathetic).

🔹 Epithalamus

  • Pineal body: pinealocytes synthesize melatonin (circadian rhythm).
  • Light → retina → suprachiasmatic → pineal pathway.
  • Parinaud syndrome (pineal tumor): upward gaze palsy, pupillary abnormalities.
  • Precocious puberty in young males with pineal lesions.
  • Habenular nuclei: part of limbic system.

🔸 Subthalamus

  • Subthalamic nucleus: component of basal ganglia circuitry.
  • Hemiballismus – contralateral flinging movements (lesion).
